High-Need High-Cost Patients

Health systems, clinicians, and patients have all identified High Need High Cost (HNHC) patients as a priority focus for population health programs. HNHC patients are a small proportion (five percent) of patients that have major health problems and account for a disproportionate share (50 percent) of health care utilization.

Using our comprehensive linkage infrastructure we integrated clinical, Medicare claims and social determinant data to develop, test, and deploy ten distinct computable phenotypes characterizing HNHC patients to national health systems and healthcare leaders.
